Signs It May Be Time to Transition to a Different PM&R Practice

Physical Medicine & Rehabilitation offers a wide range of career paths, from inpatient rehabilitation and private practice to interventional pain, sports medicine, academic medicine, and leadership roles. While many physiatrists remain with the same employer for years, there are times when exploring new opportunities may be the right career move.

Farr Healthcare has never had so many practice opportunities as in past years. Sign-on bonuses are more common, and employers are increasingly willing to wait for more than a year for a new doctor to start to help families make the move.

Here are some questions to consider before your next move:

Has Your Compensation Stopped Reflecting Your Value?

If your compensation has remained stagnant despite increased productivity, growing patient volumes, expanded responsibilities, or additional certifications, it may be time to evaluate the market. Many physiatrists discover significant differences in compensation models, bonus structures, and partnership opportunities when they explore other practices.

Is Work-Life Balance Becoming Unsustainable?

Burnout continues to be a significant concern within PM&R. The American Academy of Physical Medicine & Rehabilitation (AAPM&R) has identified physician wellness as a major priority due to increasing burnout rates among physiatrists. If excessive call schedules, administrative burdens, or productivity pressures are affecting your quality of life, a different practice setting may offer a healthier balance.

Have Your Career Goals Changed?

Many physiatrists evolve professionally over time. You may want to transition from inpatient rehabilitation to outpatient practice, add interventional procedures, pursue sports medicine, move into leadership, or reduce clinical hours. If your current employer cannot support those goals, exploring new opportunities may be appropriate.

Example: an interventional physiatrist recently asked about an SNF practice opportunity due to systemic constraints in their current work environment, including staffing shortages and a recent loss of procedural space.

Are Leadership Opportunities Limited?

Experienced physiatrists often seek opportunities to lead service lines, develop programs, mentor younger physicians, or participate in practice management. If advancement opportunities are unavailable in your current organization, another practice may provide a pathway for continued professional growth.

Do You Feel Undervalued or Disconnected?

Compensation is important, but culture matters too. Lack of physician input, poor communication, inadequate support staff, or misalignment with organizational values can contribute to long-term dissatisfaction. If these issues persist despite efforts to address them, a change may be worth considering.

Is the Market Favorable for a Transition?

The physician job market remains strong. The U.S. Bureau of Labor Statistics projects approximately 23,600 physician and surgeon openings annually through 2034, creating opportunities for experienced physicians to explore new practice settings and compensation structures.

Here are some opportunities that Farr Healthcare represents that might have incentives that stand out to you:
