PQRS 2015 – ARE YOU READY?

In 2013, Eligible Professionals (EPs) were required to report, as part of CMS’s Physician Quality Reporting System (PQRS) on one measure
for one patient. Although simple enough, many EPs failed to report, and are being penalized (CMS uses the term “adjusted”) 1.5% of Medicare reimbursements in calendar year 2015.For 2014, EPs were required to report on 3 measures for more than 50% of Medicare patients to whom the measures applied, or face a 2%
penalty on all 2016 Medicare reimbursements.For 2015, EPs are required to report on 9 measures across 3 domains, and must report on a “cross-cutting” measure. The penalty for not reporting  as indicated will be FOUR percent (2% for PQRS and 2% for VBM (Value-based Modifier) of all Medicare reimbursements in calendar year 2017.Physiatrists who have an outpatient practice may be able to report the Preventive Care Measures Group via a registry, and thereby meet the requirement.

Those physiatrists with an inpatient-only practice have other options, and may be able to take advantage of “loopholes” in the PQRS program which allow reporting of less than nine measures. The EP will be subject to MAV (Measure Applicability Validation), and may avoid the penalty, as long as the measure(s) reported were all reported for more than 50% of eligible patients, and at least one cross-cutting measure was successfully reported. When subject to MAV, the process will determine if the measure(s) reported were part of a cluster, and if so, were the other measures in the cluster reported.

For 2015, some physiatrists have “given up,” and are willing to absorb the 4% penalty, in order to avoid the headaches and administrative costs of reporting in 2015. Although I readily acknowledge that the PQRS program is becoming more complicated, I also believe that “giving up” is short-sighted because there are other short-cut options available, some of which will enable the physiatrist to meet the PQRS requirement efficiently.
