Quality Payment Program
Summary & Recommendation for Hospital-Based Physiatrists

Background
For 2017, CMS has overhauled the old PQRS reporting system, and has replaced it with the Quality Payment Program (QPP).QPP requirements were issued in the last quarter of 2016, and physiatry practices were hard pressed to learn everything required to implement the program for 2017.  Stated more bluntly, it was virtually impossible to become expert in this brand new, complex program in such a short period of time.Summary
The Quality Payment Program consists of 4 categories:
1. Quality (equivalent to old PQRS)
2. Advancing Care Information (equivalent to old EHR program)
3. Cost (not effective until 2018)
4. Improvement ActivitiesEach of these 4 categories will be given scores, which will then be weighted to
produce a composite score. Based upon this composite score, practices will receive a
positive or negative adjustment ranging from -4% to +4% in 2019.- For 2017, there is a “test option” which makes it extremely easy to meet the
requirement for 2017, and thereby avoid a penalty in 2019.
– For those practices seeking to earn an incentive, data must be reported for at least
90 days.
– For those practices without an EHR, CMS will still offer the option to obtain an
exemption from EHR requirements.Recommendation
QPP is complex, and there is a limited time frame for learning. In addition, CMS has
still not promulgated all information (EHR Exemption form still not available, and
Measure Applicability Validation options (MAV) are still not finalized). Therefore, in order
to avoid a potential penalty in 2019, it is recommended that hospital-based physiatrists
use the “test option” to meet the QPP requirement for 2017.
